You can add links in the form that connects to a separate page - like company disclaimer, etc.

To add links in the form:

1. Click Candidates tab.
2. Click Customize the candidate detail.
3. In "other fields" section, rename the field "picklist field".

To rename the picklist field:

4. Mouse over the field and click edit this field.
5. Change your Lable name as <a href="link you want to add">Read and Accept Company Disclaimer Policy</a>.
6. Add options Yes and No.
7. Click Update
